Enrollment 2021-2022

All accepted students must enroll in the Grant College Prep and Digital Arts Magnet.

​

Print and complete the following forms:

  • Student Enrollment Form

  • Emergency Information Form

  • Permanent Health History Form

  • Residency Questionnaire

  • Parent/Guardian Publicity Authorization Form

  • Magnet Mentor application (optional)

​

Make photocopies of the following documents:

  • Proof of Residency (e.g., DWP or gas bill; only one page required)

  • Parent/Guardian's Identification (e.g., driver's license or passport)

  • Student's Birth Certificate

  • Student's Current Immunization Records

  • Student's IEP or 504 documents (if applicable)

  • Student's Transcript/PAR (if applicable)

Note: Only students coming from a school outside of LAUSD need to provide a copy of their transcript.
